Snow Crows With a PvE perspective, I'm going to run through the changes, discuss their impact, and talk about current meta builds and how they're impacted.

I know this is a lot. Think of it as a reference document.
A LOT has changed. The builds aren't unrecognizable but the texture has changed almost across the board.
Flanking is Dead (for most endgame bosse)
To make them more reliable in endgame PvE content, all effects that benefit when the player strikes from the flank or from behind now always apply their benefits when striking defiant foes.
Not all players know that "flanking" or "from behind or the sides" just means—don't be in front of the target.
Ever used a build with Thief runes? Just don't stand directly in front of the target.
What fewer players will know is that there's a small set of skills that actually need you to be ~180-degrees behind your target.
- Thief Backstab (deals double damage from behind)
- Thief Sword Stealth Attack (only dazes from behind)
- Ranger Greatsword Hilt (dazes from the front, stuns from behind. This basically doesn't matter in PvE)
- Mesmer Downstate phantasm (deals more damage if the phantasm hits from behind. Hey it's like a Thief!)
Which way does Keep Construct face when it descends back to the arena after the orb-rift-push phase?
Which way does Sabetha face during her Flame Wall?
I'm not going to answer these questions because the answers don't matter anymore! This skill expression is now gone.
Just backstab from any angle.
Ah but "defiant foes"? What
isn't included there?
Can you backstab Primordus in the Harvest Temple strike if your group stays on the left vs right side, when looking from the center towards the dragon?
You would have to be on the right side. The left is the dragon's face.
Which way does Conjured Amalgamate face?
When approaching CA's platform, it is facing to your right. This also applies to the hands. Stand on the left side of the platform, and "above" the hands (closer to the portal leading away from this encounter).
Which way do the hands on Adina face?
2 o'clock (north), 5 o'clock (northwest), 7 o'clock (south), 10 o'clock (southeast)
No, I don't know why.
Just use your rifle and pierce two hands with Spotter's Shot + Death's Judgment! That's better than remembering these facing positions.
These are non-defiant "prop" type enemies. Also included here is Drakkar, the Octovine, and Tequatl. IIRC.
BTW Drakkar is made up 17 prop pieces, and you can only cast Deadeye's Mark on one of them. Don't play Deadeye here, you're trolling yourself. :D
Acrobatics
Yeah this ain't it, chief. This is still a defensive and mobility-oriented traitline.
The damage modifier in Endless Stamina can't compete with Deadly Arts or Critical Strikes, even when combined with the Power in Swinder's Equilibrium.
The other stuff is maybe interesting for solo open world champions but Shadow Arts is already quite capable at that, and Condition Thief has a lot more toys to work with when solo compared to Power.
It's something but this traitline still feels ironically directionless.
Improvisation
Interesting! We can now use this traitline predictably, resetting the cooldown of all or utility skills. On the surface that's good because it means we'll always get value out of the reset...unless our skill uses are staggered. More on that later.
Mag Bomb
Currently this skill's damage-per-cast-time is roughly double that of Daredevil Staff 2, Weakening Charge. Notably, the cast time is short because it pulses damage.
The coefficient we have on the wiki, drawn from the Game's API, is 4.10. ANet lists a 1.50 (per hit, so 4.50 total) nerfed to 0.70. So... Uh...
Let's say Throw Magnetic Bomb's damage is nerfed to 50% of its current damage. Or there about.
So next patch, one Throw Magnetic Bomb is close to the damage of one Staff 2, but the damage comes out over time (during our Assassin Signet burst) and the cast animation is faster than Staff 2.
But is it worth losing Executioner?
Probably not. Executioner is approximately 9.52% damage on a fight that spends 50% of its time above 50% target HP and 50% of time below (due to Ferocious Strikes).
That's a lot of damage to be made up by another use of a stolen skill!
Daredevil's Dodge
Bound: Increased power coefficient from 1.75 to 3.5
This ratchets up the damage of our dodge so that it beats the damage-per-cast-time of Punishing Strikes (skill 1 part C) and Staff 5, while still losing to Staff Strike and Staff Bash (skill 1 parts A/B), Staff 2, and Fist Flurry/Palm Strike.
This damage increase is valuable because
- (A) it makes dodging better for damage, on top of the damage modifiers provided by Havoc Specialist and Bounding Dodger
- (B) if we get excess endurance from our helpful allies, burning it off to maintain our modifiers hurts our DPS less on the future patch.
Auto chain go brrrr
Staff Strike, Staff Bash: Increased power coefficient.
... Uh oh.
This increase the DPS of the first and second part of the chain well beyond the last hit.
The optimal DPS rotation now involves interrupting the chain after the 2nd hit.
That's not easy to do on most professions, which have a limited number of viable interrupts, but we're Thief, baby!
Staff Strike, Staff Bash, Weakening Charge. Repeat. Throw in a Fist Flurry. Save your Palm Strike. Yeah. This sort of thing.

A not insignificant DPS increase provided by this (unintended?) balance predicament.
You could just not do this. That's up to you. The build is getting buffed by ~4,000 DPS next patch but it still has all the old problems. Don't know those?

Various Physical Skill cooldown reductions
These are neat and strictly a positive.
Left, a great player who provides benchmarks for Power and Condition Daredevil, already tested Condition Daredevil with cooldown-traited Impairing Daggers and found them simply worse in group content vs Devourer Venom. You can read more on his
benchmark.
So the Impairing Daggers cooldown reduction (CDR) doesn't matter to us.
A cooldown reduction for Channeled Vigor would've been cool!
This skill basically gives you a get-out-of-jail-free dodge if you're ever scared that burning all your dodges for Havoc Specialist will leave you vulnerable. You can get a dodge with Withdraw on a shorter cooldown but that travels backwards from your facing direction which can be scary.
This skill is also very useful for maintaining Lotus Training uptime when playing Condition Daredevil. If you can't keep hitting the boss with your auto-attack chain and F1 Steal, you will run out of endurance and lose your condition damage buff. Use Channeled Vigor to maintain buff uptime when you can't hit the boss. (This tip comes from Left!)
Impact Strike is still worse defiance-bar to cooldown ratio vs Basilisk Venom, but any improvement is nice. This skill is good when you can't guarantee all hits of Basilisk Venom will hit your target, or when you need to CC more frequently than Basilisk's cooldown allows.
Detonate Plasma and "Boon Thief"
Farewell, Boon Thief. Without Quickness, we can no longer role-compress all those shiny boons in Plasma into a Quickness Build.
I'm going to go out on a limb here and assume that the damage of Detonate Plasma will not be worth its cast time purely from a DPS perspective, especially on a Condition Build, but you might still get some boon uptime from Daredevils on fights like Matthias and Twin Largos' Kenut.
You can now permanently sunset your Celestial/Ritualist gear for Boon Thief. Will you use it for Alacrity Specter later? I don't now, I don't know how much Alacrity is given by that trait. We'll talk more later. :)
Icebrood Saga Stolen Skills
These really don't matter. Unstable Reagent is maybe a DPS increase, I don't have a log with it for cast time.
This is NOT Unstable Artifact BTW. That skill is still worth using for both Power and Condition DPS.
Cursed Artifact is like throw-Plaguelands, and an Ethereal field at that. It's free real estate on the Condition Build.
Time in a Bottle is squad-wide alacrity AND quickness. Take that, Chronomancer! It's only found in Cold War and randomly in EoD strikes. :)
Deadeye
More like dumbed-down-eye.
Deadeye Stolen Skill splash support
Stolen Skills now grant their beneficial effects around the caster.
These are mostly "splash" uptimes of boons, but you can also get superspeed and a modest heal. Neat! Deadeye providing a token amount of squad value beyond its DPS and CC output.
Deadeye Malicious Intent buff
Malicious Intent: Increased Malice gain from 1 to 2 in PvE only
This is actually big!
Firstly, this increases the damage dealt by the lower difficulty
Be Quick or Be Killed Dagger Deadeye rotation (if you've ever heard someone described Dagger Deadeye as 5111151115111, that's this).
1 more Malice pip after each Mark and Stealth attack boost the damage modifier of Malicious Backstab. I don't have a quick estimate for this increase yet.
Secondly, this is wonderful for the Dagger-Dagger Maleficent Seven rotation.
All Power Dagger skills (Heartseeker, Dancing Dagger, Cloak & Dagger) are single-damage packet skills (with the exception of Dancing Dagger when bounced from your main target, to another enemy, and back to your main target). This means they have only one chance to score a crit, and thus only one chance to earn 2 Malice instead of 1.
Power Thief derives 15% critical chance from Critical Strikes' Keen Observer trait, which demands the player stay above 75% HP. You won't have 100% uptime of this trait in many raid fights, especially if your healer is slacking. If the current Dagger Deadeye benchmark was performed at 75% player HP, without drastically altering gear to accommodate for the missing critical chance, the benchmark could lose up to 12,000 DPS!, dropping it nearly into support territory.
This Malicious Intent buff gives you a safety cushion for one of those Initiative skills to score a non-crit, making the rotation more robust in endgame encounters. It does NOT reduce the minimum number of Initiative skills needed to reach maximum Malice: that is still 3 with Malicious Intent and 4 without. It does reduce the maximum needed by 1 skill, which is also great!
Quickly Fire For Effect
This trait no longer requires a target to grant boons to allies.
Er, it doesn't require one right now. Specifically, if you Mark a target, you can use your F2 and gain boons even if your stolen skill is obstructed by terrain. If you have your enemy targeted while outside 1,500 range you will get an
out of range error that stops you from casting F2, but simply untargeting the enemy will let you use the skill to get boons. If you are obstructed or out of range, you don't hit the target.
I assume what this means is that if you do NOT have a marked target, you can cast a cantrip to get a new F2. That's cool and rather handy for the new Quickness build.
Shadow Flare isn't real, it can't hurt you
Shadow Flare: Reduced cooldown from 30 to 20 seconds. This skill only damages once instead of pulsing. Activate the flip-over Shadow Swap to trigger another damage instance from your original location.
Deadeye dumbed down. Navigating self-reveal with pulsing damage skills on DPS Deadeye has been a hallmark of modern builds for several years—Shadow Flare for Power, Thousand Needles for Condition.
That problem is now eliminated for Power. You don't need to know the timing, you don't need to know where to throw Shadow Flare. Just Do your backstab, cast Shadow Flare for one damage packet, cast it again, go for another Backstab. No fuss, no skill expression, no frustration, no learning curve.
I don't like this change, I understand why other people like this change. I think it's dumb to change this. If you don't like self-reveal, consider not playing the class that uses Stealth and Revealed for its DPS output.
One in the Chamber cute damage buff
One in the Chamber: This trait now also increase the damage of F2 Stolen Skills in addition to its previous effects
In case anyone is unfamiliar with Deadeye—it doesn't have access to any core Stolen Skills. No Mag Bombs here!
This is a nice perk. The important thing is that this trait gives us an extra F2 charge every time we use a Stolen Skill...That's for Quickness.
This trait is also valuable to Condition Deadeye builds (particularly since there's a 4/9 chance they get a stolen skill that applies damaging conditions!) and to Power Pistol-Pistol Deadeye. See more of that

Binding Shadow, my beloved
Binding Shadow - Reduced cooldown from 30 to20 seconds in PvE.
Cool! I love—
This skill now immobilizes marked targets instead of knocking them down
Oh. Uhhh it already immobilized them. I assume it immobilizes them more? That sucks, this was an amazing breakbar skill, particular on Condition Deadeye where it applied several poison stacks (natively, and via Panic Strike with Immobilize) and gave an extra F2 charge from One in the Chamber.
It's lower cooldown on boonstrip which is neat? Matches No Pain No Gain time in Fractals, I suppose.
Just use a Power Mesmer, they passively breathe boonstrip.
Quickness Deadeye builds
Three options!
Oops! All cantrips This option will spam out cantrip like nobody's business to meet the voracious demand of Might uptime.
Fire For Effect grants 8 stacks of Might for 12 seconds. The with-Alacrity cooldown of Cantrips are: 16 (Binding Shadow, Shadow Flare), 20 seconds (Malicious Restoration heal), 25 seconds (Mercy, Shadow Gust). When using Mercy, the cooldown of F1 is effectively the cooldown of Mercy, plus minimally the cast time of the F2, plus the cast time of F1 again.
So some of our Might is locked behind 25+ seconds. Building for 100% Might Duration would be devastating for our DPS output. We could cover 100% Might uptime with a single trio of F2 casts anyway—Binding Shadow, Malicious Restoration, Shadow Flare—but that would not take tactical advantage of the natural Deadeye's Mark cooldown.
Instead, we'll cover Quickness with two sets of F2 casts, staggered across the duration of our Might stacks. Left has an old benchmark demonstrating this technique.
So that's our 24-might upkeep option. It's not pretty, and it's Cantrip spam. ANet said we wouldn't have this in the game anymore.
Well, what if we don't have to generate Might?
Then we have two options:
3 cantrips with no Improvisation This option will maintain a burst of 24 might as we apply quickness, or we can space out the stolen skills to maintain at least 8 stacks indefinitely. Sounds pretty good!
Improvisation We'll probably still be using Shadow Flare because it's just good damage, and Improvisation selects for builds that use active utility skills, not passive signets. We might even be able to use Malicious Intent instead of One in the Chamber for this build, earning us more damage on Malicious Backstab.
So we have options.
Quickness Deadeye weapon options (We don't have those! )
Maleficent Seven is the only equalizer at work that allows bad weapons to do somewhat okay, and without that, we're back to shopping with the to the usual harsh restriction of "the best single skill out of all these weapons crushes the validity of all other options".
Stealth attacks? Naw, just spam F2s without Malice.
"Filler" initiative skills used to build up to an Initiative reset at max Malice? Nope, don't care, whatever has the best initiative-to-damage ratio is what we're running.
Thief Dagger is about matched with Thief sword for auto attack damage but Dagger is home to the centralizing Cloak & Dagger → Backstab combo. This does huge damage from any direction—flanking is gone!
Sword, by contrast, suffers a DPS loss when using any dual wield skill, Headshot, or Black Powder. The only DPS gain skills are Cloak & Dagger (but the stealth attack for sword is BARELY worth using, you could actually just cast Cloak & Dagger twice in a row and do just as well), and Sword 2...without using the return-to shadowstep. Meaning Sword 2 has a 15-second cooldown. Just to do slightly more than the auto-attack.
Need CC? Pack an off-hand pistol and fill time with auto-attacks, they're very good.
What about Pistol?
It's just weaker.
Shortbow?
It sucks really bad against single targets.
Rifle? This is the sniper spec!
It sucks if you don't have Maleficent Seven for infinite Initiative and either stealth utility skills or Silent Scope to unlock Death's Judgment.
Specter, rant about single-target/ally-target support
Ally-targeted scepter skills now also grant a lesser effect to additional allies in a radius around the target.
I don't know why ANet is so obsessed with single target support for PvE Specter, but they'll learn its untenable eventually. They've been gradually walking back the single target focus ever since release, and we are now about 15 months past Specter's official release and coming up on 2 years since it's beta.
Ally-targeting is clunky.
Single-target support is nonviable. We have encounters that apply arena damage to all players, and each player in the squad can take damage from the same AoE and it's the healer's job to keep the whole team up.
A "lesser effect". Reduced duration? Great, what's the point of that? You might as well just make it all the reduced duration.
Does "splashed" Shadow Sap not even grant protection to the other allies? Great, we won't use it.
Hopefully this will just be something like reduced Barrier or Healing, and the Boons are intact, but it's already been confirmed by the new member of the design team, Trig, on a Twitch Stream (Mighty Teapot's?) that Endless Night will not give Quickness to "splashed" allies.
Maybe this skill shouldn't give quickness at all. Why is the design team not skill splitting? What are they afraid of?
The Dark Side of Scepter Splash
If you paid attention to Specter during its beta testing, you might be aware of a
very weird build that used Endless Night (Scepter Pistol dual wield beam skill that granted quickness and 7-packets of Barrier to 3 allies, if you could pierce through the allies).
This rotation was initially theorycrafted by Left when Specter was first showed off. I did some early proof-of-concept on the build, then Left took it over and improved it to around 51,000 DPS.
That's a lot of damage!
Watch that video briefly, and notice how I swap between the enemy and my allies, applying one skill to the target then the Endless Night beam to my allies.
When Specter applies barrier to an ally, they receive a stack of Rotwallow Venom, which applies a short-duration stack of torment to a single enemy just like normal venoms.
Each cast of Endless Night back then was 21 applications of Rotwallow Venom, with more Torment duration than we have now.
ANet killed that build back during beta by making the beam target a single enemy, nerfing the quickness application, nerfing Rotwallow Venom duration, nerfing Specter's Torment damage modifier, nerfing Consume Shadows to require charging up.... The list goes on.
Why do I bring this up?
Scepter Autos on Allies
Currently the scepter auto chain applies 54 seconds of Torment (in the form of multiple stacks). If the scepter auto chain can instead apply barrier to 3 allies, then this balloons to 72 seconds without Strength of Shadows, or 108 seconds with Strength of Shadows.
Yes.
The auto-attack chain will be twice as strong when targeting allies, vs targeting the enemy.
Endless Night is not worth using against an enemy target, but it might be worth using on allies. This rotation won't have room for auto attacks, so I'm not pinning any hopes to this.
No, the optimal damage rotation will probably be Twilight combo, Siphon, and Shroud skills on the enemy, then filling time with Scepter skills on allies. We will probably spend less time in Shroud because those autos are just so cracked...
Second Opinion, moving away from Consume Shadows
Ever since beta all forms of Specter have run Consume Shadows—Alacrity, DPS, Heal. This is because Consume Shadows can apply barrier to allies, giving them Rotwallow venom. The other adept traits are purely defensive, so they're not used by the DPS and Alacrity builds...and Consume Shadows is an extremely potent healing tool. So potent, it's been nerfed repeatedly!
Charged time reduced from instant to 4 seconds, maximum shroud cut by more than 50%, conversion ratio reduced from 100 to 50%
Second Opinion will grant bonus Condition Damage, more with a Scepter. This should be, at minimum, +80/+80, which will beat the ~800 DPS provided by perfect Rotwallow Venom application from Consume Shadows.
Perfect?
If your allies are too injured and Consume Shadows only heals them overflowing into Barrier, you won't give them Rotwallow.
Traversing Dusk no longer gives Alacrity
This is just a heal trait now.
Shadestep is the new Alacrity trait
Oh. Uh. I'll keep it quick.
- This reduces our gameplay from 2 separate rotations with their own quirks and different Shroud lengths, to 1—the DPS rotation.
How do you give alacrity? Just go into shroud and push buttons.
How do you do DPS? Just go into shroud and push buttons.
Alacrity Specter's Well cooldowns were inaccessible inside Shadow Shroud because we inherited a
shroud, not just a kit like Druid's Celestial Avatar or Holosmith's Photon Forge.
Druid Spirits off cooldown while in CA? Don't care, you can push em.
Specter wells off cooldown while in Shadow Shroud? You might be gaining DPS from shroud skills, or healing from Consume Shadows, but you're losing Alacrity uptime. Is that tradeoff worth it?
Doesn't matter anymore. Just do the benchmark DPS rotation.
You're too slow? Then your alacrity uptime will suffer. You need to be on pace with going in and out of shroud, just like the DPS rotation. Difference is, if you're bad at this, it's going to frustrate other people.
This is not the hardest rotation in game to optimize and you of course have the option to make it easier with Ritualist gear (which you should do anyway if you plan to still run Consume Shadows, will be a two-fold personal DPS loss).
- Ever been kicked out of Shroud by incoming damage in a raid/strike/open world meta? No more Alacrity uptime for you. Better refill your Shadow Force quickly—and Larcenous Torment generates Shadow Force FAR more slowly than Traversing Dusk
Fun fact: Traversing Dusk's 1% Shadow Force per ally in your shadowstep scales indefinitely, not just up to 5 players. That gives you colossal Shadow Force generation in crowded metas and even raids.
Want to use your Siphon on ally? No you don't, that's valuable Shadow Force generation you need to maintain tempo for Alacrity.
Again, you can wear Ritualist gear to soften this loss.
Want to have the utility of Well of Bounty? Well, previously that utility was tied to your Alacrity uptime, your DPS uptime (lingering outside shroud for Well of Bounty was a loss), and your healing output (Traversing Dusk, and maintaining tempo in/out of Shadow Shroud for Consume Shadows).
Now you can take Well of Bounty and have it on demand, at a DPS loss.
How much DPS will ANet allow this build to have?
We don't know. And now it's DPS is very closely tied to the DPS build because they're doing nearly the exact same thing.

Unchanged by Impairing Daggers cooldown reduction
Power Daredevil
Buffed to ~38.6 if you do the old rotation.
Buffed to 40.2 if you do the new degenerate 1a 1b 2 1a 1b 2 rotation that interrupts our auto chain.
Still screwed by all the usual stuff.
Mag Bomb damage normalized. Cool. Still amazing utility on that skill.
Boon Thief
Dead. Deadeye killed it.
Power Dagger Deadeye
Much easier to play. Splashes some cute boons onto its subgroup, at random. Have to see if Shadow Flare is still worth using. Hopefully, right?
Power Rifle Deadeye
Silent Scope uses Shadow Flare, I hope that's still worth using.
No need to flank with Premeditation Rifle is nice. Swapping Impact to an Accuracy sigil (or not doing that, and losing crit chance) was ~1,200 DPS loss, so that's avoided. I don't mind losing flanking here that much because Flanking wasn't make-or-break adrenaline pumping like dagger. It was just a boring loss.
Quickness Deadeye
I hope you like pushing 5 and 1 a whole lot because that's almost all this is. Also pushing cantrips for F2 charges to provide quickness.
It will play okay, but pretttttttttty similar to utility-spam builds that people hate, with the added bonus of casting TWO skills for every application of quickness.
You can tune this to your liking BUT
IF YOU DON'T KNOW HOW MANY WAYS THIS GAME'S ENEMIES CAN APPLY REVEALED TO YOU THEN YOU'RE IN FOR A TREAT WHEN YOU PICK UP QUICKNESS DEADEYE NEXT PATCH :D
It won't really prevent you from applying quickness but it will tank your DPS and fracture your soul.

This is getting no changes, by the way. It'd be a huge amount of effort from the studio's programmers for one elite spec, for one profession, largely in one game mode (instanced PvE), with a small player base.
Probably just not happening. Not a question of "want to", it's a question of resource management AND doing this right.
Remember stealth-tanking Old Lion's Court? Because they didn't apply Revealed, then they only applied Revealed once and Deadeye just Shadow Melded out of it?
DPS Specter
Buffed by Second Opinion, dubiously buffed by targeting allies with the auto chain.
You can still run Consume Shadows for team healing/barrier.
Alacrity Specter
I don't know. Presumably the shroud skills will allow us to provide at least 50% uptime with a non-degenerate rotation (one that doesn't have us spending all our time in shroud). Said degenerate rotation is naturally policed by the fact our shroud absorbs incoming damage.
Will it run Consume Shadows? Not for optimal damage anymore.
Alac Share Specter
Just change your grandmaster from Strength of Shadows to Shadestep and do the normal rotation. Should do at least 50% uptime, unless Alac specter is just nonviable and can't maintain Alacrity.

Hey guys. Kind of curious to know why I wasn't hired. I know I can't get a definitive answer here but I would love to hear what everyone here thinks.
There's a panda down the street from where I live and they've always got hiring signs. My whole working life, I've always worked in either sales, tech support, call centers that kind of thing and I wanted to try something different and so I applied back in February. No response until today. I completely forgot I applied but I was sent an email said "After careful consideration, we regret to inform you that you have not been selected for this position. " I applied for the basic Kitchen & Service position. I'm curious what qualifications am I missing? I don't have a criminal record or anything. Paid Employment - Not Medical/Clinical: Clerk @ Internal Med Clinic - Ok so for this job it was like 5% clinical because I took vitals. Does this totally eliminate it from it being non-clinical? 95% of the time I was fighting insurance to pay for a patient's medication (prior auths) and inputting explanation of benefits info into our EHR to balance out the posted patient charges. and calling insurance for verification. and sending out referrals to coordinate care.

| so i'm 10 days post op and have kind of been struggling for the last couple of days with health anxiety for literally no good reason lol but overall i am so so happy ! on day 8 i noticed some funky discharge on my dressings and convinced myself i had an infection which they were very quick to get back to me and reassure me that it didn't look like an infection and that i was a ok for that point in recovery, & was confirmed to be totally ok on day 9 when i had my dressings removed. at that appointment i almost fainted seeing my incisions & raw nips for the first time lol. i spent most of yesterday convincing myself that my nipple was falling off DESPITE the team reassuring me everything looks good and then me telling myself that the lil puckering wrinkling on my incisions is a sign i'm stunting my healing and just in general my health anxiety has been at 100 for the last 3 days. i'd prepared myself for post op depression but honestly the closest i've felt to that is just exhaustion from having to constantly meter my anxiety, and feeling a bit lonely in the whole process as i don't think my parents want to see my incisions (they are wonderful and looking after me otherwise!) and my partner has shown not a lot of interest in anything relating to my surgery so i've been feeling a bit alone with processing everything (hence me writing an essay for this post lol thanks guys <3). i'm the only person i know who's actually seen my chest this is all a preface however to the fact that i KNOW this is all anxiety and me problems lol because i am so so so pleased with my results & how they look even this early on. somehow even the swelling looks good on me lol like i don't know why i am fixating on all these literal non issues when the team has said everything is good healing wise and i am sooo chuffed with the aesthetic outcome!! literally i cannot sing chloe wright's praises enough. not only does she have a bright & reassuring manner but i could not have asked for a better contour. i honestly didn't have great aesthetic expectations because i've spent so long hating my body whether from dysphoria or issues with my weight but ever since i first saw my chest out of its post op binder all that's just melted away, like the feeling of catharsis and peace with my body is like nothing i've ever experienced before. i can't explain it but the exact placement of my scars and nips just feels so incredibly /right/. even though at first i was really worried about how i'd feel about these things i feel like she has just like. known lol exactly where things need to be. i just keep being like huh guess i'm cis now! lol tl;dr is dam that Miss Wright can do a surgery ! she is great and communicative as well, really great at talking me through things before hand and answering any questions and concerns i had. she also answered all 3 of my silly little emails within a couple of hours, even at like 8 pm on a friday the whole entire team have been amazing in fact, i have met both the other surgeons (one for my pre op and one for my post op) and they both seemed wonderful too, again super reassuring and respectful. i saw stacie the nurse at both my pre op and post op and she was also amazing and gave amazing and thorough advice for after care and preparation. in fact the whole team seems so well informed and passionate about providing the best care they can, i really can't recommend them enough :) anyway this is my little essay over, i just wanted to vent slightly lol but also share how happy i am and also share my results as i've been staring at these pics every 5 seconds to reassure myself anyway lmao thank you very much for reading if you managed to wade through! 2023.06.10 03:15 RafflesiaArnoldii The Defense Mechanisms of Type 1
(In case you didn’t see my edits to the first post, I decided to split these by type so they’re more digestable. Also, though I happened to do 1 first, I won’t necessarily do them ‘in order’.)
justification
Naranjo explained this explicitly but folded it under part of reaction formation, however I think it’s distinct enough to be discussed on its own – BHE also discussed this in a podcast episode without assigning it a name.
What happens is, the 1 has strong ideas and preferences about how things ‘should’ be and how they want their life to go in a regular, orderly manner – starting from the part of the reptile brain that is to do with regularity & sense of order, and then building on it further with their search for “the best way to do things” and the experience-based judgements of their implicit mind.
Much of the goal behind this is autonomy – Like the child that gets trusted with more decisions because they are so mature & responsible & don’t give others reason to scold them.
But what if someone comes along & tells you to do things differently anyway?
You argue that your way is right, whether that means its more efficient, more rational, more moral, more responsible etc. and so by extension theirs is wrong.
In this, the rational mind may well be employed to great effect (many 1s are doctors, engineers, philosophers etc.) but sort of in the way that philosophy was employed in the middle ages: Any paths of reasoning that could contradict “what is right” (in the case of the medieval philosophers, church doctrine) weren’t going to be pursued.
So if you have an 1 who prefers to get up early, they might espouse beliefs in the advantages of getting up early & how this is the most disciplined & virtuous way to do this, whereas if another 1 is a night owl they will embark on a mission to make workplaces more friedly to people with different circadian rythms, and neither might connect it to their own preference -
Instead they will completely frame it in impersonal terms of competency and/or superego – this way is most efficient, this way is correct, this way is what’s right. Or: That thing is objectively wrong & disgusting and anyone who doesn’t see that is a sick fuck. You’re not nitpicking, you care that it’s done right.
When others get the impression that you’re just trying to boss them & force your way on them, you don’t see where it’s coming from – it’s not about your way, you think, it’s about “what’s right”… so you figure it must be because they’re debauched or something.
Useful Question:
If you strongly feel that something is right: Is it truly the best method, or is it maybe just the way that you personally prefer it?
Note also that realizing you simply prefer it doesn’t mean you have to give up or self-flagellate for being selfish or failing to be impartial.
In some situations you might have an easier time getting your partner to agree to consider your needs & preferences if you say it personally bothers you rather than argue about the objectively correct way to load a dishwasher.
It’s ok to have wants, preferences & be subjective in some areas. Obvsly if you’re doing surgery you should totally follow the protocol as perfectly as possible, but ‘correct’ clothes or hairstyle is much harder to define.
You can prefer something even if it’s not “Objectively The Best”. It just ‘feels’ less ‘defensible’ because as a competency type you have a neutrality bias. And of course once you allow that it becomes natural to let others have different preferences.
Meanwhile, if you strongly feel something is wrong: Why is it wrong? Get that head center online. Now if you’re not used to it, this can be uncomfortable: 1s have a high need for closure and hence dislike ambiguity – they tend to want a clear answer, now. When you’re used to relying on the implicit mind whose output feels very solid, real and obvious, going into the abstractions of the mental center can feel airy & ungrounded – you might be worried that you might come up with an excuse or talk yourself into thinking some terrible thing is actually ok. But your implicit mind can mislead you as much as abstract theory might.
You might find that forbidding a lot of what’s universally agreed to be awful can easily be justified in terms of logical reasons. Look at the 6s – you can use your head center and still be morals-oriented. Be more willing to consider logical counterarguments even for things that trigger repulsion in you – just considering them without per se accepting them. Even if you still disagree, knowing how the other side thinks can help you persuade them.
reaction formation
But what if there is a desire that cannot be justified in this way, perhaps because it clashes with the 1s stated values, principles and beliefs? You can’t easily give up on the principles if those feel integral to who you are, why you do everything you do & why you should get anything at all.
So, the incongruent impulse is transformed into its opposite.
To consider why it has to be like this, it might be illustrative to tell you about a discussion I read on here once of various 8s (whose default tendency is to act on most impulses in an unmodified form) on how to overcome problems with impulsivity. Just trying to hold in the impulse through sheer will often doesn’t work because the next thing will just set them off, there is a quick connection between stimulus & response, thinking their way out of it like a head type might won’t cut it, but what does work a lot better is to point the ‘kinetic energy’ somewhere else, such as by exercising.
The gut types take in the world on a viscerally felt, kinesthetic level (Rohr, a 1 himself, referred to it as ‘full body blows’) & this leads to building up immediate reactions to it. So ‘redirecting’ the impulse, or rather forming an immediate counter-reaction to it, is more natural than just extinguishing it.
The purpose of the reaction is not only to “defend” against acting the ‘unacceptable’ impulse, but to “defend” your conscious self from the distress and incongrence of even recognizing the impulse, that is, to push it out of consciousness. Like saying some prayers to make the “dirty thoughts” go away.
If you read the experiences of 1s (or even 1 fixers) you see that the impulses already arrive in consciousness in this ‘civilized’ form: Not ‘this annoys me’ but ‘this is not good enough!’
The physically felt swell of what might otherwise be irritation comes in as frustration about how the thing was done wrong & how it could all be perfect if only the co-workers bothered to be a little more dilligent.
This is part of why experiencing an actual unmodified impulse can feel so threatening or shameful.
First they would be experienced as unwanted, ego-dystonic, incongruent with the self.
A rabit horndog is not who you are… and certainly not who you want to be.
There is not so much previous experience of thinking ‘I kinda want to do violence to this jerk’, or of having a horny thought… and then not acting on it for a variety of reasons.
So you might fear that if you loosened up you would actually be violent or sexually inappropriate. (which isn’t helped if the only time you cut loose if when you got so much pent up frustration you can’t hold it in anymore, so you overcompensate like a person going on a binge after an overly restrictive diet.)
What this may concretely look like is being extra polite to someone who lowkey irritates you (disowning the “bad”/”unresonable” feeling), or condemning something that you actually find tempting on some level –
Tom Condon described the case of a type 1 friend who liked to collect rocks. One time they came across a cool-looking amethyst geode in a shop window and the friend’s eyes lit up, but then he saw the hefty price tag and at once he started ranting about how ppl these days waste so much money on frivollous impulse buys. (his desire for the rock had conflicted with his value of being thrifty, so a reaction against it was formed.)
Condon then said something like ‘You really want that rock, huh?’
In this case the friend had the self-awareness to admit it, but with some immature 1s (or ppl with strong 1 components), they can get weirdly obsessed with the thing they’re condemning & the lives of people who are doing it. ‘Oh this fetish porn is so horrible!’ ...why do you know so much about it, mate?
(part of it is also that by confronting the “bad” and rejecting it, the sensation of your boundaries holding firm against the outside, you feel your existence, so when you are having self-doubt…
It’s the frustration thing of using “not that!” as orientation.)
This is how you get ppl complaining about people taking breaks, gender non-conformity, childfree couples, divorce, more empathetic rather than strict parenting etc. because how dare those ppl not give up the desires you renouced?
Ppl complaining about divorce have the unhappiest marriages, ppl complaining about the “selfishness” of the childfree are some of the most emotionally distant parents and so on.
Maybe you remember that horrible “marriage requires a mute button” article a while back whre that woman tried to sell you on the idea that every responsible adult must accept that marriage is constantly being irritated at your spouse. (with zero self-awareness of how she described constantly nitpicking, criticizing & failing to show empathy for the husband, & refusing to communicate her wants/needs)
When ppl responded to it saying they actually like their spouses or pointing out that divorce is a thing, she accused them of being naive because A Mature Person(TM) is supposed to make it work and accept that marriage isn’t a cakewalk, damnit.
Reading it you might wonder “why is she torturing herself?” but probably the value of “making a marriage work through realism” is very important to her.
Useful Question: Could it be that this thing bothers you because you kinda want it but think you’re not allowed to have it?
Now for the love of sanity, don’t go torturing yourself by thinking you must secretly want everything that grosses you out. This question is mostly useful to ask when you find yourself disproportionally fixating on something. Being outraged about child labor is probably just because child labor is outrageous, but if it’s a minor lifetime choice thing that won’t kill anyone, it might be worth considering.
suppression
The next ‘layer’ of defense, if incongruous desire or anger does leak out in its ‘raw’ form, is to tamp it down asap & deliberately suppress it.
You try not to be angry or desireous.
As gut types experience feelings mostly through body sensations so suppressing them also means counteracting the bodily impulse that comes with them – this is sometimes described as feeling like the rising heat of anger gets stopped at your neck & jaw, like a cork on a champagne bottle, or the relaxing sensation of pleasure is stopped by tightening & straightening the spine & pelvis.
So there can be actual physical tension held in those areas.
This also feeds into the self-judgement, because a suppressed reaction still happened and you still feel it, even if you put a stop to it.
In that sense 1s might be uniquely cursed among the competency types. You’d physically feel the anger or desire & the need to stop it. So someone might beat themselves up for even thinking of getting angry.
Another thing to note is that while this might seem at first like a relatively voluntary/ semi-conscious action, but when some 1s try to deliberately express their feelings more, they sometimes find the habit to clamp it down has become quite automatic.
- an extreme case of this might be ppl raised in very conservative towns who were told to suppress all sexual desire until marriage, but then when they’re finally married they’re too tense to have enjoyable relations.
Unhealthy 1s can get quite dour & joyless cause pleasure is suspect & suppressed.
- though healthier 1s will be able to relax & express feelings when appropiate, but still be able to restrain themselves where this is useful, like if you’re a surgeon or an emergency responder & need to keep your cool & keep doing the task. In that setting “defending” against getting irritated or otherwise emotional is arguably a good thing, at least for the duration of the important work, it allows 1s to be level-headed professionals.
Useful Question: Being patient, disciplined & restrained is often very useful, but are you counterbalancing this with some outlets? (dance, time in nature, and regular vacations seem to be some popular ones with well-adjusted type 1s)
Reparation & undoing
This is sort of a cousin of reaction formation in that it also involves “making up” for a “bad” thought/deep by doing something “good”, but the difference is that in this case, the horses have already left the barn, rather than preventing an incongruent thought or action these are about coping with the aftermath after it has already occurred.
The above constellation (particularly on the less aware/mature side) can easily produce a lot of guilt that then needs to be ‘defended’ against lest it set off the fear of condemnation.
Undoing is to take an opposite action immediately afterwards, which in the person’s mind makes it so the “bad” thought or deed never happened.
For example you thought about blowing up at someone and then acted extra nice to them because you felt bad about it, or you actually did blow up at your spouse but then buy them some flowers to “make it okay”.
In the case of reparation, the bad deed is not quite considered ‘erased’, it still ‘counts’ and there are still feelings of guilt and accountablity, but you made up for it. You’re a changed man.
This is like those people telling you about the wicked life they used to live before they came to jesus, but now they are completely different.
Sometimes making amends or focussing on doing a better job next time is, of course, a constructive way to deal with mistakes. (and 1s for their part also forgive more easily if they see someone’s willing to make amends, have consequences, show they learned something etc.)
There are two ways this can become problematic in less aware individuals, however.
First, when it becomes a compulsion to constantly make up for perceived ‘guilt’.
Cracked and ate a chocolate muffin? Now you must do 20 extra minutes of exercise!
There was an interesting anecdote on a podcast about an 1 woman who got involved with a more ‘wild’ guy, and then immediately chopped off her hair to something more austere looking, like she’d strayed too much to the wild side & needed to correct it.
The guy took it as mixed signals.
The Second is when the person considers their deed ‘made up for’, but others don’t. The wife doesn’t forgive you despite the flowers, because you can’t really erase what happened or the concerns she has with you.
Or the preacher guy who ‘came to Jesus’ expects to be forgiven by everyone now because he “repented and made amends”, “overcome his struggle with temptation” & that was “his old life”.
Another way this may look is someone considering that their spouse & kids shouldn’t bring up past grievances anymore because they apologized and promised to change.
others might well find it hypocritical when someone who has clearly messed up at some point in his life starts talking to you about the Best Way To Do Things and how you should be following their sage advice. They won’t just act as if the past actions never happened.
There is, of course, a strong incentive to believe in an espouse the redemption narative when it feels like the only other alternative is that you are an Irredeemable Bad Person(TM) forever. Trying harder to strive for the perfect world that ought to have been is one’s saving grace – if that doesn’t count, then what does?
So as understandable it is that others might call bull/hypocrisy, there is also a lot of pain behind this which, alas, often only causes the counterproductive patterns to become more rigid.
Someone’s response being “try harder!” (for all that it can be the 1s greatest strength in other contexts) can also make it harder to recognize situations where it would be better to walk away or stop trying to do the impossible.
Useful Question:
On the one hand: Will anything good come out of continuing to be harsh on yourself? Maybe you can forgive and/or accept yourself without feeling like you need to constantly work on “redemption” or “betterment” – especially if the cause of your feeling guilty is just being an imperfect fallible human.
On the other hand, especially if your actions impacted another: Does your idea of what would constitute restituition align with the wronged party?

Supreme Court cases do not operate the way the Independent article suggests. Small details in a decision, tangential to the issue being decided, do not turn it into a landmark case, or even establish precedent. Rather, it is a decision's holding which determines whether a case is a landmark or not. The Independent is foreign owned and operated, but being a large outlet, they should know better than to put out an article like the one you linked to.
In the court system, pronoun policy is currently split at the appellate level, with the Sixth Circuit and the Seventh Circuit taking opposing sides. The Supreme Court has not weighed in on this yet, but the split makes it more likely they will choose to rule about it.
Besides this, Jonathon Turley writes at

Both Champaign and Indianapolis are large cities, which can draw on outlying areas to get workers, making up for having below-replacement fertility. Door County also relies on importing workers, but because it is not a large city with lots of political power, this is less reliable. In 2020, for example, the supply of foreign J-1 workers was abruptly cut off. Door County manages to get a consider number of immigrants from outside the county, but most who relocate for longer than a season are wealthier than people residing in Door County. They also tend to be older and not looking to start new families.
Instead, as larger-scale governing bodies destabilize Door County and break down its social fabric and economy, they can use former Door Countyites to solve the worker shortages in large metro areas. Support for transgenderism is a shibboleth for the economic interests of large metro areas. This manifests itself locally as a lower quality of life and an economic leakage to outside the county. Because it isn't a big city, Door County is especially dependent on its birth rate for its future. Its total fertility rate in 2020 was 1.533 lifetime births, well below replacement of 2.1 lifetime births. At this rate, each generation, without factoring for immigration or emigration, will be 27% smaller than the one before. However, since something like 30% to 40% of young adults leave the county, the effect is even stronger than 27%.
Transgenderism's concomitant drugs and surgeries tend towards sterilization, and towards making its subjects less attractive to the opposite sex. Assisted reproductive technologies are too expensive to be implemented on a broad scale. Transgenderism is a risk to Door County's future, because it could help push that 1.533 figure even lower. In this way, transgenderism is a demographic factor in the overall gentrification, just as unaffordable housing is an economic factor. Just as it is unwise to encourage unaffordable housing, it is unwise to encourage transgenderism.
Or even if you think replacement fertility is unrealistic and unachievable, consider that when the age structure of a population gets too unbalanced, it is harder to take care of the elderly. The struggles at Scandia Village are an example of this:

It follows then, that someone who refuses to use preferred pronouns might not be a horrible bigot who gets perverse satisfaction from it, but rather, someone who loves his or her community and wants it to have a future. Such people may even be willing to personally be on the losing side of a litmus test for the collective good.

Some of them, such as the Miller Art Museum, Door County Community Auditorium, Peninsula Music Festival, Northern Sky Theater, have benefited from, and can expect to consider benefiting from, public, tax-funded support. Because taxpayers come from all sorts of political backgrounds, these organizations should have avoided all political activities, aside from lobbying for particular things directly related to their organization.

Institutions which benefit from public funding should be non-partisan and impartial. Public money should not be spent on group-think.
Most of the sponsors are local, but Carrie Busse Shorewest Realtors and Napalese are from Green Bay, the Avenue Art & Company is from Appleton, and Marketini Media is from Madison. They get to influence Door County while their cities benefit from Door County's emigrating young-adult population.
I'm not sure if this has been covered in your classes so far, or privately by your boss. If so, bear with me as I restate it. The Pulse has a different political makeup among its advertisers versus its readers. That it is a free paper tends to result in a readership which is inherently less educated than the readership of a subscription newspaper. I used to know a man who had once ran a free paper, but sold it. He was a conservative. It had a lot of general-interest content, nothing political, and especially featured ads for alcohol. Free papers tend to be read by people who read occasionally rather than regularly.
Occasional readers are less educated than regular readers, and the level of education corresponds to the political alignment. This means that natural readership of the Pulse is more conservative than the overall population of Door County. For people with a regular reading habit, the Pulse competes with a number of subscription outlets, while for occasional readers the Pulse has less competition.
Yet the Pulse isn't dominated by gas station and liquor store ads. Unlike many other free papers, its advertisers run more towards the left. This makes sense, because presently Door County has a disproportionately left-wing visitorship. One factor in the loss of right-leaning tourists may be in marketing decisions over recent decades has shifted. Right-wingers from the Chicago metro area are probably drawn more towards state parks in Indiana and other Door County alternatives. Local businesses geared toward tourists, or even seasonal residents, will probably be well-served by advertising in the Pulse.
This leaves the Pulse in the situation where it is to the left of its readership, but still financially sustainable. Its leadership must be aware of this vulnerability, as the Pulse's podcast tends to be to the left of the paper. That it sometimes leaves its readership behind is exemplified how, in the recent midterm election, Ron Johnson did not return any answers to the Pulse's questionnaire, while his Democratic opponent did. Door Countyites voted for him anyway, by a margin of only 75 votes. During the 2022 election it seems that a number of Republicans intentionally spurned media outlets over left-wing bias. To the extent that they did so, and won office anyway, this served to send a message to their base that they were unwilling to be manipulated by left-wing media. It could even be seen as a plea for fair treatment in the future.
Instead of pronouns, there are other topics out there. Recently, a large firm was paid a lot of state money to write a report which claimed that "Potawatomi was chosen in honor of the tribe that had lived in the area before ceding its land to the US government in the 1830’s."

I think it would be well-received locally, though maybe not by the DNR. Had you written an article about how pronouns should always be biologically honest, that would have hurt your chances of getting a job someday as a journalist in a large metro area, but I don't think debunking Graef would hurt you. With some effort, you should be able to find other topics which are win-win for both your future career and for Door County. Although this is a social relationship, were it a biological relationship instead, this would be "mutualism" rather than "parasitism" or "predation".

My family members are always telling me that there is "something" wrong with me. I feel that I've done my very best to improve myself and I am genuinely happy with my life but I've grown up with a lot of trauma (caused by said family) so I question my reality, often. I'm worried that I'm part of an unhealthy dynamic with family, and I may not be aware of it. My friends tell me that it could ruin or hinder my development like it has in the past.
I genuinely don't know if I'm doing ok. Maybe I think it's ok but in reality - I'm not ok. I feel better and I laugh, smile and think more positively. My friends have warned me that it seems as though I am in a narcissistic family of which they have designated me as their scapegoat. The reason for believeing this (and I agree with them to an extent) is that when I share normal healthy news about my life - my family says it's the opposite. Maybe my friends are right, and self improvement may involve leaving this unhealthy dynamic.
For example, all throughout my childhood, I was fed really bad food. Fast food, dollar store candy for breakfast, chips for dinner, etc. I ate whatever was lying around. I now eat a very healthy diet but struggled a bit due to binge eating disorder. I went to therapy for my disorder, joined an OA group and I'm doing great. I exercise regular, eat 2 meals a day with a light snack and focus on sleep and meditation. I also had hygiene problems because my parents didn't bathe me often. My schools were always getting involved - it was a mess. I was neglected for days at a time because my parents would leave me by myself in our apartment. I had to walk myself to school, feed myself, and tuck myself in.
But I'm doing great now. I focus on the things that I didn't have as a kid and I'm sober (alcohol & drugs). I no longer have a craving to eat those foods, stay up all hours of the night, not shower for weeks on end because of depressive thoughts, etc but my families response is : I have low self esteem because I'm always trying to fix my looks through dieting and I'm stuck up for trying to eat a healthy diet that is too expensive and is just a waste of money. I am a boring person whose sad and self centered on the inside and I try to find meaning in a useless existence because I never go anywhere or do anything (in their opinion).
I can't seem to catch a break, really. If I'm trying to save money and reduce my spending - it's because I'm broke and unsuccessful in life. If I make plans to travel - it's because I must be running from my problems at home. If I don't smile every second of the day while I'm with them- it's because I'm depressed. If I defend myself - it's because I'm an aggressive and overly sensitive person and I need to grow thicker skin. I'm just not good at anything. But if other family members do these things, my folks treat them like they're amazing.
I don't want to be the kind of person that's always blaming their problems on family. I also don't feel that my family and I are "happy" as a family unit. I don't want to let myself become comfortable with the pain. It mostly hurts me now as an adult because I feel like I will never have the family that I wanted but I understand that taking care of myself from here on out - is my responsibility and that I should protect and defend that, at all costs.
I want to continue to succeed and live a fullfilling life but I can't shake this feeling that a lot of my backsliding happens because I continue to surround myself with family but family doesn't have my best interest in mind. I guess I feel like I could improve myself a lot easier without their negativity and judgement, in my life. Maybe my strength going forward, will require me to break this unhealthy dynamic and leave? I don't live with them, but I mean like to distance myself from them. What do you guys think?

u/ThatYoungBusinessGuy for being very informative and supportive throughout my journey, providing me information that I would never have been able to obtain otherwise, even through extensive research online.
Discovery of varicocele: Found out I had varicocele years ago, when I was about 12-14. I had a sudden pain in my left testicle that would only go away when I lifted my legs above my head. I looked online and had an inkling that it might be varicocele, but the pain just would not go away. Unfortunately, after I told my parents they panicked and took me to the ER, as we thought it might've been something like torsion. They gave me an US, confirmed that it was varicocele, and sent me home.
Progression of varicocele: It started out G1/G2, only able to be felt. It soon progressed to G3 over time, yet it never really gave me any problems. Very infrequent pain (once every month for a few minutes at most), even after lifting. I would mention my varicocele every annual visit with my primary care doctor, yet they would mention each time that if it wasn't giving me any pain, it would be fine as its main effects were decreased fertility.
My varicocele would always be there, though. A huge constant bulge when standing up/showering/going to the bathroom, and a soft, squishy mess when lying down. Last year, in Fall 2022, my primary physician did give me the information of a urologist she recommends that works at the University of Washington, an amazing hospital/health center in Washington near Seattle. However, because my varicocele wasn't giving me many problems, I just went on with my life.
Urgency in fixing: In March/April this year, it started giving me pain. Like,
really bad pain, 6-7/10, which I've never felt from my varicocele before. My job included standing for long periods of time, so it made my time hell. This persisted for a week or so, and because I was going out of state for college in September 2023, I started to want to get my condition fixed as fast as possible. I scheduled a meeting in March/April with the urologist at UW my primary care doctor gave me. However, I was going out of town in late June and leaving for college in mid August, and because my first consultation could only be scheduled in late June with him, I scheduled a meeting with another urologist at another hospital, Virginia Mason.
Virginia Mason meeting: I had my meeting in early April. Coming in, I wasn't set on whether I wanted surgery or embo. I knew I had a tight schedule, but I was willing to hear what my uro had to say.
He mentioned how he does the operation around 15 times a year, which was a cause of concern for me (you can check my post on my profile). On one hand, I wanted to get this over with, but on the other hand, I didn't really trust 15/year that much.
After having some doubts and wanting some clarification on the procedure (because he was kinda vague during our meeting), I scheduled a phone meeting with him on a Tuesday in mid April. He cleared up some specifics, but I still had a few concerns. I eventually decided to ask for a referral to an IR at UW to see if I could get embo.
IR meeting: My meeting was scheduled for mid May. The IR explained it extremely clearly and was very kind. However, he kinda strawmanned the surgical approach (he claimed that the surgery would litigate the symptomatic veins in the scrotum, which is one type of procedure, but isn't the microsurgery which is what most urologists do).
He explained that he would embolize the entire gonadal vein (which is the ISV in males). He would do it with the sandwich method, where he would use coils and then a sclerosant foam, which would irritate and close of the vein while also trickling down into side veins and also irritating them. He mentioned that it would be more painful than using only coils, but might be more effective. I was extremely confident in him as although he only did this varicocele embolization surgery around 15 times a year, he does IR embolization in other body parts many many times per year. Very collected, calm, and experienced. We scheduled my embo for June 8th, which was just yesterday.
Operation day: Checked in at 6:00am, got called in to prepare at 7:00am. They set up an IV, got me in a gown, and took my blood pressure. I met the nurse and IR before he went off to a 7:30 meeting. The nurse debriefed me, told me about the procedure/healing, gave me antibiotics through the IV, and told me that I would be given some calming medication and fentanyl for pain.
At 8:00am, I was wheeled into the operation room. There were two techs and two nurses at first. They got me prepped and hooked up to monitors, moved me onto the operating table, and hooked up my IV. They were all friendly and energetic people, joking around the entire time. The lead tech shaved my incision site (right groin area) and they set up all the radiation protection. I kept bringing my head up to look around (which was not something I was supposed to do, as it flexes the abdominal muscles) so the nurse gave me a thick cloth to prop my head up.
At around 8:30 the IR came in with a resident and a 4th year med student, they numbed the incision site, and set up a screen next to me which showed what the IR was seeing. I'm not 100% sure, but I think I was under a fluoroscopy machine (kinda like XRay?) where I could only distinguish my spine.
The screen next to me was meant for the doctors to look at when they turned around. I was able to watch the entire thing on the screen next to me. They first maneuvered the catheter to my gonadal vein, then guided a straight metal rod through the catheter. Once they reached what I assumed was the end of my ISV before it branched into the pampiniform plexus. There, they started coiling up the metal rod into coils. In the end, there were three large clumps of metal which consisted of all the coils (overall, there were 10-12 coils), with space in between where they put the sclerosant foam. Halfway through the procedure I could feel pain in my lower abdominal area, so the nurse gave me an extra dose of fentanyl.
Recovery: I was scheduled to be in recover for 2-3 hours until 12:30pm, but I stayed there until around 3:00pm. When I was supposed to be discharged at 12:30pm, my heart rate dropped considerably (high 30s to low 40s) and my blood pressure dropped to 100 something. However, these issues were mainly due to hydration, and after hooking me up to saline for a few hours I was able to leave. The entire time, I was told to keep my right leg straight to prevent the entry vein from rupturing.
When I first left, pain on my left side was around a 2-3, while my incision site was around a 3-5 in pain level. The pain in my left side was mainly due to the sclerosant foam, which would leak from my ISV and cause general pain and irritation in the area. When I got home, I just lay down on my couch/bed the entire day.
As I'm typing this in my chair the next day, pain is 0-1 for both my left and right side. The incision site is still giving a bit of a problem when I walk around/stretch the area, but my left side is giving me the most problem. The pain on my flank/lower abdominal area is 0-1 normally, but a 1-2 when walking or coughing.
Varicocele status: I feel so much better. It might just be placebo, but my left testicle is feeling much cooler than before. When standing, the varicocele is considerably smaller and feels hard. Considering that it takes 3-5 months for my body to adjust and redirect blood flow, the improvement after one day is incredible. No more fat ugly bulge when going to the bathroom, no more distant, aching pain. I was even surprised at how I am able to subconsciously notice a difference - I was used to feeling my underwear against my scrotum on the left side as a result of my enlarged veins, but with the reduction in size, I'm surprised at how much empty space there is and how much smaller my veins are.
Sorry for the long story/poor grammar. I quickly typed this up and just wanted to share my story here, especially because this subreddit (and especially YoungBusinessGuy) was a huge source of support and information for me. I'll provide updates after a few months (I have an ultrasound scheduled for Aug 8), but for now, I feel much better. I'm feeling possibly dilated veins on my right side, so I might be back, but hopefully I'll be able to put this in the past.

My(24f) Husbands(25m) uncle (FIL’s brother) passed away 3 years ago. His family didn’t have a funeral, The uncle had mental health issues so he didn’t have anyone in his life outside of immediate family and myself. In February of this year we get this random text that on June 3(no significance to this date) there’s going to be a funeral for him in Ohio. We agreed to go(it’s only 3 hours away from us and we would be back home that night) even though it’s really weird this is happening almost 3 years later on an insignificant date. We agreed to go despite our feelings of it being 3 years later strictly out of respect for FIL.
In March, we went on a trip to Virginia for a wedding and we stayed with his parents. His dad brought up that he’s booking an Airbnb for the funeral and we have to stay with them. This was ignored by both my husband and I since our dog has been having these weird stomach problems(she’s mostly back to normal now) and our pet sitter had to clean up shit and bathe her for 2 days. It kept getting worse she couldn’t stop throwing up or control her bowel movements. We left our trip early while our angel of a pet sitter stayed with her until we got home. At this point with us leaving early, FIL is upset because he wanted us to spend the night in Ohio, and he was trying to book an Airbnb, even though we never agreed to that and he was like “oh, I guess I shouldn’t have asked for that given the circumstances with the dog”. 24 hours later, he still booked the Airbnb.
Anyway, so throughout April and May our dog was still having these issues where we couldn’t leave the house for more than eight hours without her having uncontrollable diarrhea/vomit for multiple days. My husband tells his mother that we’re not gonna be able to spend the night because of our dog and we don’t want our pet sitter to have to deal with what he went through in March again because that would not be fair to him and not fair to the dog to lay in her own shit overnight. MIL just started screaming at husband on the phone saying that “this is funeral more important than our dog and we should have our pet sitter watch her so we can stay the night with them and be a family.” This controversial conversation went on for weeks until they finally let up on our decision to prioritize our dogs health. This past Friday, inlaws dog ended up needing emergency surgery. So they had to reschedule this funeral, which is okay however we’re moving back to to our home state in the south in a few weeks and we have a lot going on. I suggested the first weekend of July if they want us to be able to come this summer, but that doesn’t work for them because there’s gonna be traffic and they have a vacation the second week of July. I also suggested a date in two weeks from now and heard crickets. They decided that we weren’t going to go and they decided on a date where I said that my mom I haven’t seen in six months(she just became a flight attendant and it’s the only days she has off she will be flying from Detroit to see me and then back to Detroit for work) is coming to visit and help me pack and husband didn’t want to takeoff of work for this 3 year late funeral as the emotions/urgency has been gone since then. Husband gets screamed at by his mother saying how important this is to his dad and just going off on him and suggesting my husband just leaves me at home and he comes. Then SIL(22f) calls and screams at him that this is important and my “plans are bullshit and everyone is saying it’s bullshit”. They get into it but nothing is resolved. FIL and Ben were chatting about other dates on the phone and SIL comes in screaming because she has a babysitting job on a Friday/Thursday that would work. That phone call ends abruptly and MIL calls husband to scream at him and then the FIL and SIL chime in to start screaming at him to leave me at home and why can’t I be left alone with my mom(my mom was coming to visit with both of us and help us) and why can’t I pack by myself(I have pretty bad health issues). I finally yelled in their presence for the first time “I don’t fucking care anymore just go without me so SIL can go to her fucking babysitting job I don’t even want to go anymore” then they all shut the fuck up and I pretended to leave the room and then SIL starts crying on the phone & now everyone is upset. Next day FIL calls husband and said he understands if husband doesn’t come as my feelings were extremely hurt and this is an open wound they keep opening. Alright so, husband called FIL (who said previously he understands if Ben doesn’t go to stand by me) to tell FIL he’s not going and FIL started screaming at my husband. Apparently they are all offended/embarrassed I yelled obscenities at SIL. After that, my husband was upset and said he has to go to this funeral, I agreed but I’m still upset over how I have been treated like a pet you can just leave at home. Anyways I ended up calling FIL and told him how I feel, it was going okay but then he was saying he doesn’t let his children cuss at eachother and then he randomly asked if my father deceased 10 years ago) allowed that and I stated that my dad encouraged us to fight it out and get over it. He then said that SIL apologized. In my head, I was like to what the wind? I left the room. He was almost insinuating I need to apologize and I stated that I would not be apologizing. He then asked if I wanted to be treated like his daughter because if I want that that means he would tell me to get over it and I would have to come to the funeral regardless of my plans. Found that comment to be bizarre as he’s not my father nor my fathers replacement. MIL had tried calling me that day but I didn’t answer bc I am not in the right headspace and he told me he would’ve told me to pick up the damn phone if I was his daughter…. But I’m not your daughter I’m an adult who’s married to your son. Anyways that phone call ended with me saying I need to be left alone right now and he agreed.
MIL texted me afterwards saying she’s upset and wants to talk to me. Husband called inlaws and was pissed that she texted me. I responded to her with “I am not ready to talk right now cause my emotions are high as well and I want to be able to have a productive conversation with you.”
Anyways fast forward to yesterday MIL said to husband on a phone call that she doesn’t understand why I am offended/hurt. He explained that I am his wife, I was one of the few people that knew his uncle outside of family and they have disrespected me multiple times previously in similar situations. Husband told me he explained to MIL his priorities were with me and she apparently said ”what if your priorities change” and also said “well you’re going without her now anyways so why is she upset”
This has been exhausting. Of course if this death had recently happened, we would both drop anything and everything for it but as it can be rescheduled, I’m not understanding why it has to be a specific time that ONLY works for them.
